How do i create a dvd cover that will fit onto a cd slimline case.?

Instead of using blank dvd cases i have seen people make covers that fit onto cd cases. It just has the artwork and a review of the movie but I don't know how it was done. Please help. And I don't need to hear about how it is illegal to make duplicate moviesHow do i create a dvd cover that will fit onto a cd slimline case.?
Scan the original at 250dpi (for clarity and print readability)on a flat bed scanner ;then print it on to 40 - 65 pound card stock. Fold, trim slip it into the CD case
